JOB DESCRIPTION

Mental Health Liaison Coordinator

Complex Needs Team

Grade: NJC scale points 23 - 25

Hours: 18.5 hours per week ( 2days a week, flexibility essential)

Responsible to: Team Leader Head of Complex Needs / Chief Executive GASPED.

Employing Body: GASPED Wakefield

Location: Wakefield Pinderfields Hospital

BACKGROUND

The Complex Needs team provide support in Mid Yorkshire Hospitals Teaching NHS Trust to patients who have/may have complex needs (learning disability, autism, dementia, delirium, or mental illness).

Patients requiring support in hospital or following discharge may have complex needs and require, or would benefit from, appropriate support in the community.

In doing this we aim to avoid delays in patient discharge and to ensure that they have the best possible chance of achieving stability and maximising their independence in the community.

PURPOSE OF THE ROLE

The post holder will have a high degree of autonomy and will be supported by the Complex Needs team and GASPED. The post holder will be expected to be flexible and proactive in developing and shaping it. Being able to drive is desirable.

You will work alongside the Complex Needs team to identify patients who require support. The post holder will identify any support needs the patient may have in the community. They will proactively engage with community health teams, community mental health services, complex needs team, primary care colleagues and family/carers to coordinate the relevant support that may be required in the community.

The post holder will work with Mid Yorkshire Teaching NHS Trust’s Complex Needs Delivery Group to support them to meet the targets identified in the group’s work plan and the complex needs 5-year strategy.

PURPOSE OF SERVICE

To ensure patients with complex needs have their mental health needs identified and support provided in the community. The role will take account of any specific needs in relation to marginalised groups including LGBTQIA+, BME and disability.

Being sensitive to the needs of disadvantaged groups including LGBTQIA+ and BME communities in the planning and delivery of services and interventions; using co-production, promoting independence, and social inclusion.

To support the transition from hospital to community for patients with complex needs over the age of 16.
To ensure needs are assessed, and plans include the provision of support or referral to suitable services in the community. In conjunction with risk assessments, maintain close links with other services in line with Safeguarding procedures and make other appropriate interventions and escalation.
Work as part of a Multidisciplinary Team to manage a proportional response to risk.
Provide advice to patients, carers, families and professionals.
Promote positive partnerships, collaborative working, establish and maintain close and effective relationships with all stakeholders within Mid Yorkshire Teaching NHS Trust and third sector partners.
The main focus of the role will be to improve the mental health of patients that regularly attend Mid Yorkshire Teaching NHS Trust and to improve their engagement with community services. This will ensure that they have the best possible chance of achieving stability and maximising their independence in the community.

KEY RESPONSIBILITIES

Clinical

To coordinate care in the discharge planning for patients that have complex needs.
To support patients by working through any barriers identified following individual assessment.
To deliver a range of effective, person centred interventions aimed at promoting recovery
Supporting patients to adopt strategies that promote wellbeing, healthy lifestyle, independence and self-care
To manage risk safely and effectively to improve the wellbeing of those with complex needs.
Coordinate care and risk planning for patients in the community and in the hospital setting, bridging the gap between hospital and community services.
To ensure that care is carefully and meaningfully coordinated in order to ensure the patient is supported.
To operate within the legal requirements of the Mental Health Act and Mental Capacity Act.
Actively coordinate health education
Provide relevant support to carers and families in relation to their rights
Have a good awareness of services available in the third sector and utilise

as needed to support patients in their recovery.

12. Support the service in gaining patient and carer feedback which will help

Inform service change and development.

Communication

Communicate accurate information to patients and families on the support available for patients with complex needs
Act as an advocate for patients with complex needs
Maintain working links with other agencies to ensure smooth transitions of care for patients.
Liaise and communicate effectively with community and primary care and social care teams
Participate in service evaluations, benchmarking, research and audit activities
Contribute to implementation of service modernisation and improvement projects
To support a culture of learning and continuous service improvement amongst the team

Professional, Legal and Educational

To coordinate specialist assessments and interventions in line with the professional training.
To share knowledge with team members through inter-professional training

GENERAL

Attend a full induction programme consisting of development and wellbeing planning, goal setting, shadowing, and an introductory support training course.
Be responsible for personal learning and development and undertake appropriate training, both mandatory and optional to increase knowledge, skills and awareness.
Attend regular support sessions, reflective practice, supervision and performance reviews with your GASPED CEO and Complex Needs team line manager, in line with the organisation’s performance management policies and procedures.
Work at all times as part of a team. This includes working with other staff, attending team and staff meetings and developing a teamwork approach to all aspects of the organisation’s work.
Be flexible in the approach to hours of work including evening and weekend work as required by the service.
Be aware of and employ the general practices of Mid Yorkshire Teaching NHS Trust/GASPED regarding safeguarding and health and safety policies and ensure these are adhered to at all times.
Operate within the aims, policies, and practices of Mid Yorkshire Teaching NHS Trust/GASPED at all times and to be committed to and promote the organisation’s equal opportunities and anti-discriminatory policies.
Ensure information is dealt with in accordance with Mid Yorkshire Teaching NHS Trust/GASPED policies around confidentiality, communications, internet, e-mail and telecommunications, and steps are taken to ensure that confidential information is secure e.g. service user data.
To ensure that Data Protection, Health & Safety, Complaints Handling and Corporate Governance requirements are met.
Keep the GASPED CEO and Complex Needs Team Manager informed about any serious and untoward incidents, safeguarding, health and safety, and/or financial issues.
Undertake any other duties as directed by your Line manager and Complex Needs Team Manager in line with the responsibilities of the post.
