About The Role

About the Role

We are looking for a proactive and organised Transition & Resettlement Coordinator to support post-18 young people as they move towards independent living across Bedspace regions. The role involves coordinating transition plans, managing a caseload, and working closely with internal teams and external agencies to achieve positive outcomes for young people.

This role is Full-time, Permanent working 37.5 hours Monday – Friday, it requires a full driving licence, and you'll have your own vehicle that is insured for business use. This role includes working from either the Manchester or Liverpool Office, infrequent travel to our other locations may be required at times across Manchester, Preston, Liverpool and Leeds areas. The annual salary for this role is £27,795.09 per annum in line with our annual salary review this role will increase to £29,452.59 as of 1st August 2026.

About You

You will have experience supporting vulnerable young people or individuals with complex needs and an understanding of issues including mental health, trauma, substance misuse, and offending behaviour.

You will be organised, professional, and confident managing both direct support and administrative responsibilities. Strong communication, partnership working, and IT skills are essential, alongside a positive and values-driven approach.

Key Responsibilities

Manage and coordinate transition and move-on plans for young people
Conduct monthly property inspections and escalate concerns where required
Chair progress meetings and work alongside support teams during transition periods
Support with tenancy setup, budgeting, utilities, and housing applications
Assist with benefits claims and external appointments
Liaise with Housing, Maintenance, and Accounts teams to support positive outcomes
Maintain accurate records and ensure compliance with policies and procedures

Essential Requirements

Experience within housing, residential support, youth work, or similar services
Excellent communication, organisation, and decision-making skills
Ability to support individuals with complex needs
Full driving licence and access to a vehicle insured for business use
Strong administrative and IT skills
Willingness to complete an Enhanced DBS check
