Wellbeing Advisor - Romsey and North Baddesley (Hampshire)
£10,959.00 p.a based on 15 hours per week (Monday and Wednesday)
As one of Hampshire’s leading Mental Health Charities, Andover Mind is entering an exciting phase of growth to meet the needs of our communities around mental health and wellbeing support associated with the Community NHS Transformation Programme.
The mental health and wellbeing of the communities across Hampshire is our top priority and we are looking for a Wellbeing Advisor to join a growing mental health service across the Romsey and North Baddesley Primary Care Network. Primary Care Networks (PCNs) are groups of GP practices who work together in a local geographical area and over the last couple of years, we have been working with the NHS to improve the local mental health services within GP surgeries in our region.
The overall purpose of this role will be to directly assess patients from Romsey and North Baddesley GP surgeries who are referred into the Primary Care Mental Health service. You will also provide short-term recovery-focussed intervention to patients on ways they can manage and improve their mental health through teaching them coping skills, using resources and referring into other services when appropriate.
Work is carried out in the community and in GP surgeries. This is a non-clinical role which promotes self help and works as an integral part of a growing Primary Care Mental Health Service. You will be working in a multi-disciplinary team alongside other professionals such as; peer support workers, psychological therapists, social prescribers and a dedicated GP Lead.
Key tasks and responsibilities
Be the first port of call for GPs in seeking support for managing their patients with mental health presentations.
Conduct a thorough assessment of needs and wellbeing on patients referred to the service.
Develop care plans for those individuals with identified needs.
Refer to secondary care mental health services where appropriate.
Help people to focus on achievable goals using a strengths based approach.
Provide self-management tools relevant to an individual’s condition.
Hours will mainly be worked in the GP surgeries, with some option for home working and some hours may also be worked at the Andover Wellbeing Centres.
If you are empathetic, organised and great with people, we would love to hear from you.
