Full job description

The Neuropsychiatry Outpatients’ Team has great pleasure in seeking a Band 4 Assistant Practitioner (AHP) to join our existing team, based at Walkergate Park, Newcastle Upon Tyne. This post provides a rare and exciting opportunity to join an established Multidisciplinary Team working in the sometimes challenging but rewarding specialism of Neuropsychiatry. You will work closely with Community Practitioners, Occupational Therapy, Physiotherapy and Speech and Language Therapy colleagues. We are a regional service covering as a minimum, the North East of England, including Northumberland, County Durham, Tyne and Wear, Cumbria and a small part of North Yorkshire.

As this post involves travelling to various locations and lone community-based working, you must be able to meet the mobility and geographical requirements of the role. Our service is currently provided from Monday to Friday 09:00 – 17:00 (excluding Bank Holidays). The post is part time at 30 hours per week.

You will provide community-based person-centered therapeutic programmes of care with people who have a Neuropsychiatric condition, including, but not exclusively, Traumatic and Acquired Brain Injury, Huntington’s Disease, Tourette’s, Parkinson’s and Epilepsy.

You will require the ability to work autonomously, manage your diary, implement, follow and update plans of care, which have been developed in collaboration with service users, families/carers, and members of the Multidisciplinary Team.

You will have responsibility for making onward referrals to other services and liaising with appropriate regional organisations in placing orders for equipment to meet Service User needs. You will also support carers by completing specific carer documentation and placing onward referrals for carers assessments.

You will need to demonstrate excellent interpersonal and organisational skills, be IT literate, and have a flexible approach to the role to meet the changing service demands as the service model develops.

You must have a strong commitment to the delivery of high quality, safe patient care and be a role model in reflecting the Trusts vision and core values.

Under the direction of registered Community Practitioners/Allied Health Professionals/Team Lead the post holder will:

Assist the Community Practitioners, Therapists and Team Lead to plan, design and develop therapy and intervention activities within the objectives of an individual’s care and treatment plan.

Contribute observations of the physical, communication and occupational barriers and strengths of the individuals based on a foundation theoretical knowledge of communication, language and occupation.

Promote Community Practitioner and Allied Health Professional recommendations within the team, support the Neuropsychiatry team in developing and facilitating the patients’ communication/interaction and occupational needs.

Use specialist technical and creative skills to facilitate the preparation and implementation of AHP occupation/communication focused tools and approaches as a member of the Multidisciplinary Team to promote recovery, wellbeing and maximise independence.

Deliver 1:1 and group treatment to patients monitoring their progress and making minor adaptations as required within sessions.

Assist Community Practitioners and Allied Health Professional Therapists in the provision of training courses to others.

Complete routine administration tasks related to general clinical support in line with departmental standards, including the maintenance, storage and archiving of patient notes.

Actively work to enhance and develop personal skills utilising a programme of Continuous Professional Development, supervision and trust appraisal.
