A fantastic opportunity has opened for a passionate and dedicated Band 5 Community Nurse to join our dynamic Community Nursing Team serving the beautiful rural locality of South West Shropshire. The post is for 22.5 hours per week. This is your chance to make a meaningful impact by delivering high‑quality, person‑centred care to patients in their own homes and local communities.

Please note this position is community based, requiring the successful applicant to see patients in their own homes. Therefore, a valid driving licence with access to a vehicle for work purposes is essential.

Working amidst rolling countryside, close‑knit villages, and a strong sense of local identity, you’ll play a vital role in providing planned nursing care that supports independence, promotes wellbeing, and enhances quality of life. As a Band 5 Community Nurse in the beautiful rural locality of South West Shropshire, you’ll deliver compassionate, high‑quality care to people in their own homes, bringing autonomy, variety, and real purpose to your day.

What You’ll Be Doing:
Delivering holistic nursing care that helps patients stay at home, avoid unnecessary hospital admissions, and achieve safe, timely discharge.

Working as an autonomous practitioner across a diverse and rewarding caseload, providing key clinical interventions.

Hours will be worked over a 7‑day service, 8am–6pm. Flexible working options available.
