Full job description

We are seeking a compassionate and proactive male or female experienced support practitioner to support a young man in his 30s with complex care & support needs within his own home, in the Haddington area.

The young man we are recruiting for has a very active lifestyle, he enjoys attending discos. He enjoys activities such as swimming, attending social groups, going for walks, cycling using adapted equipment and outdoor activities. The individual has his own mobility car that he needs to access activities and further his independence and social circle.

The individual is a happy and sociable individual. Keeping regular contact with his family and friends is crucial to his health and wellbeing.

Details

Full induction, enhanced shadowing and training provided. Support shifts are full days including sleepovers - Example – 10:00-23:00 – Sleepover – 07:00-10:00 Finish.

Support requirements – The individual requires full support with personal care, meal preparation, household support, attending health appointments in collaboration with family. Medication management, social and emotional support, a full driving license is required & willingness to use the provided mobility car.

You will be part of a small team, who have supported the individual for a number of years and you will build positive and open communication with the individuals family ensuring they are kept in regular contact, providing updates, sharing concerns or other relevant information to ensure the family remains informed and involved in any decision relating to the individuals care and support as well as ensuring the family relationship is maintained for the individuals wellbeing. You would support the individual to live the full life, widening relationships, experiences, working on goals, outcomes and aspirations.

SVQ opportunities on completion of the probationary period

Desirable experience with supporting someone

Autism, Learning Disability, Epilepsy, Coffin Lowry Syndrome and Visual Impairment, Alternative communication methods.

Successful candidate

- Driver

- Someone who has an active lifestyle.

- Ideally previous experience in supporting someone with complex care and support needs.

- Ability to adapt communication style to meet the needs of the individual.

- Skilled in building trusting and professional relationships with individuals and families.

- Ability to recognise and report concerns appropriately

- Committed to promoting independence, choice and quality of life.
