Job Summary -
We are looking for a motivated and reliable individual to join our Ageing Well team and support the delivery of our Community Asset Groups and Memory Hubs. These groups enable people living with long-term health conditions such as Dementia, Sickle Cell, Diabetes, Heart Failure, Fibromyalgia, Acquired Brain Injury, and their Carers to stay well, stay connected, and feel supported.
This is a rewarding and responsible role, ideal for someone with experience in health and social care, group facilitation, or community-based work. We also welcome applicants who bring lived experience of long-term health conditions or caring responsibilities, as this valuable insight brings important perspective to the support that we provide.
In this role, you will:
· Set up, lead, and promote group sessions for people with long-term conditions, including Dementia, and their Carers.
· Support the delivery of services across North and West Northamptonshire. Whilst we will always seek to plan travel and workloads efficiently, flexibility to work across the county is essential to ensure carers and people we support continue to receive high-quality services, particularly during periods of staff absence and vacancies.
· Work collaboratively with partners in health, social care, and the voluntary and community sector.
· Deliver engaging, informative, and meaningful activities that support people to live well and independently for longer.
· Provide one-to-one support, either by phone or face-to-face, tailored to individual needs
· Support and work alongside volunteers and peer supporters, many of whom bring valuable lived experience.
· Represent our service at community and professional events, building awareness and community connections.
We are actively expanding our Community Asset Groups across Northamptonshire and play a key role in shaping and responding to both local and county-wide support. This role is part of a multi-disciplinary, multi-agency approach, and involves working closely with professionals, volunteers, and community members. Job Description
· Plan, develop, and deliver structured, engaging group sessions and support programmes for people living with dementia or long-term health conditions and their carers across community settings in Northamptonshire.
· Coordinate all aspects of group delivery, including liaising with venues, speakers, partners, and suppliers; managing bookings and schedules; preparing resources, presentations, and activities; and ensuring sessions run smoothly, safely, and to time.
· Facilitate group discussions and provide one-to-one support, offering guidance on practical, emotional, and relational challenges affecting individuals, couples, and families
· Provide professional, person-centred information, advice, and guidance that promotes wellbeing, independence, quality of life, and empowerment, including early identification of potential crises and the development of coping strategies.
· Identify individual needs and signpost or refer people to appropriate services, while managing referrals to and from partner agencies.
· Work collaboratively with colleagues, volunteers, and external partners to design, review, and deliver inclusive, meaningful social and therapeutic programmes.
· Build and maintain effective partnerships and trusting relationships with health and social care professionals, community organisations, and other stakeholders.
· Promote equality, diversity, and inclusion by co-producing accessible services and addressing barriers experienced by underrepresented communities.
· Maintain accurate records, databases, and reports in line with organisational, safeguarding, confidentiality, and information governance requirements; evaluate activities and contribute to monitoring and reporting processes.
· Take responsibility for health, safety, and wellbeing, including completing risk assessments and complying with all relevant policies and procedures.
· Work to agreed action plans, targets, budgets, and timescales, managing a varied and sometimes complex workload.
· Contribute to wider service delivery, including helpline/support line provision and cross-service support where required.
· Participate in supervision, training, reflective practice, and continuous professional development.
· Contribute positively as part of the wider team, including attending meetings, training, and organisational activities.
· Carry out additional duties consistent with the scope, purpose, and grade of the role.
