Persons at Heightened Risk Case Worker

Cheshire Fire & Rescue Service Prevention Department

Starting Salary of £31,537 within the Salary Scale x range £31,537 - £33,699 per annum based on experience

37 hours per week

An opportunity has arisen within the Prevention department for a professional and enthusiastic Person’s at Heightened Risk Case Worker.

In this role you will need an interest in working with those who are most vulnerable or at heightened risk from fire within our communities. You will have an ability to engage with those that are hardest to reach, a keen eye for detail and the ability to evaluate options and be able to influence and encourage change. Additionally, you will be the first point of contact for colleagues providing expert guidance and will need to use your initiative and work independently and part of a team.

You will have strong verbal and written communications skills and be confident in providing advice at all levels internally and externally. This is a fast-paced environment requiring flexibility and prioritisation to meet deadlines.

This role involves regular engagement with vulnerable and at-risk individuals in their homes and within community settings. The successful candidate will be comfortable working independently, including lone working, and travelling throughout Cheshire and surrounding areas.

The position may involve exposure to sensitive and emotionally challenging situations, requiring empathy, resilience, and professionalism. Regular travel forms a significant part of the role, and a full driving licence is essential.

Manage the full lifecycle of Prevention and Protection at Home (PaHR) cases, including assessment, intervention, monitoring, and closure, for individuals at increased risk of death, serious injury, or significant loss due to fire, lifestyle factors, physical or mental health conditions, or substance misuse.

Deliver targeted prevention and safety initiatives to residents across Cheshire, Halton, and Warrington, helping to reduce risk and improve wellbeing within local communities.

Work collaboratively with partner agencies, Prevention teams, and operational colleagues to identify vulnerable individuals and coordinate appropriate support and interventions.

Triage referrals received by referral partners, assessing individual needs and determining the most suitable pathway of support to ensure positive outcomes.

Develop and maintain effective relationships with referral partners, providing training, guidance, and education to enhance understanding of prevention services and referral processes.

Safeguard adults, children, and young people who may be at risk, making referrals to relevant agencies and managing cases in line with safeguarding procedures and best practice.
