An exciting opportunity has arisen for an experienced B6 Children's Community Nurse to join a well established team. This is a 1 year fixed term post to cover maternity leave. The team cover the Surrey Downs area and benefit from being an outreach team from Epsom and St. Helier NHS Trust. This enables access to the acute services and divisional staff. The job summary:
- Be responsible for the on-going assessment, development, implementation and evaluation of programmes of specialised nursing care for babies, children, young people (BCYP) and their families/carers in the community via autonomous caseload management.
- Be required to provide high quality, evidence-based nursing care meeting a diverse range of needs within the community setting to BCYP and their families/carers.
- Contribute to clinical expertise within the team, acting as a role model in the delivery of patient care.
- Demonstrate leadership, support, supervision and teaching, contributing to the development of the service.
- Be required to act in a manner consistent with the NMC Code of Professional Conduct carrying out their duties in accordance with Trust policies and procedures.
Provide effective liaison with the multi-disciplinary team and other professional colleagues and agencies involved within the BCYP’s care.

Deliver high‑quality, evidence‑based community nursing care for BCYP in line with Trust policies, clinical governance, infection control, safeguarding, health and safety, data protection and the NMC Code. Work autonomously to manage caseloads, complete holistic assessments and home visits, recognise changes in clinical condition, provide appropriate interventions and escalate concerns. Undertake specialist nursing procedures, administer medication safely and support discharge planning with hospitals, GPs, schools, specialist teams and external agencies. Provide education, guidance and emotional support to CYP, families and carers promoting independence, wellbeing and continuity of care, including contributing to palliative and end‑of‑life support.
Maintain accurate documentation, consent, risk assessments and safe equipment use, reporting concerns promptly. Contribute to audits, supervision, appraisals, complaints handling and service improvement to maintain high standards of care and continuous quality development.
Provide leadership, support and clinical guidance to junior staff and students, acting as a professional role model promoting equality, diversity, inclusion and effective team working. Communicate effectively within the MDT and contribute to service development. Maintain professional competence through mandatory training, CPD, supervision, appraisal and NMC revalidation, sharing learning to support the development of the CCN Service.
