The Neurological Community Team is looking for an enthusiastic Band 4 Rehabilitation Assistant to work within the stroke pathway.
The successful candidate will:
Work within a supportive multidisciplinary team
Hold your own caseload of patients who have suffered an acute stroke, providing assessment and treatment for the Patients rehabilitation journey.
Implement treatment plans based around the Patients own goals.
Be based at Queen Victoria Memorial Hospital within the beautiful Welwyn countryside, but the team also remote works to support efficiency.
Be part of a fully integrated multidisciplinary team which includes Occupational Therapists, Physiotherapists, Speech and Language Therapists, Psychologists, Nurses and Rehabilitation Assistants
Have clear structures in place to support your CPD and to provide support and supervision
Have a full valid driving license and access to a car to use during your working day for business purposes is essential (unless you have a disability as defined by the Equality Act 2010).

As a Band 4 Rehabilitation Assistant you will be working within a multi-disciplinary team of health and social care professionals, providing appropriate assessments and rehabilitation to patients with neurological conditions.
Additional responsibilities include;
To maintain patient treatment records using Systmone.
To undertake a clinical caseload using knowledge and skills from theoretical and practical experience.
To work without direct supervision on own caseload within a specified clinical area undertaking tasks delegated by a qualified clinician.
To act as a first contact in certain situations.
To participate in clinical supervision, and service-level education to ensure that clinical practice is maintained to a high standard
