Ready for a New Challenge? Join Us as a Discharge Coordinator!

Sherwood Forest Hospitals are looking for dedicated and proactive Discharge Coordinators to join our dynamic team. This is your opportunity to play a vital role in patient care and hospital flow.

What You’ll Do:
Support the Discharge to Assess process, ensuring safe and timely discharges.
Help maintain flow and capacity across the Trust.
Work across all clinical areas and provide cover at King’s Mill Hospital, Mansfield Community Hospital, and Newark Hospital.

What We’re Looking For:
Excellent communication skills and a calm, adaptable approach.
Self-motivated and able to respond quickly to changing situations.
A clear understanding of discharge pathways and awareness of social care and health processes (full training provided).
A full driving licence is essential as you’ll travel between sites.

Working Pattern:
Seven-day service, including bank holidays.

Shifts include 8:00 am –4:00 pm and 8:30 am–4:30 pm.
