Job Introduction
As a Band 6 Virtual Ward Team Sister/Charge Nurse, you will join our innovative Virtual Ward Service, helping patients receive high-quality care in the place they call home. This is an exciting opportunity for an experienced nurse who is passionate about delivering proactive, patient-centred care while taking on leadership responsibilities within a supportive multidisciplinary team. You will manage a caseload of patients with complex health needs, frailty and long-term conditions, helping to prevent unnecessary hospital admissions and supporting patients to remain independent for longer. Working closely with GPs, community services, specialist teams and social care colleagues, you will play a key role in coordinating care, improving patient outcomes and shaping the future of Virtual Ward services.

The service operates across seven days, including weekends and bank holidays, offering a varied and dynamic working environment.
In this role, you will manage a defined Virtual Ward caseload, providing assessment, planning, coordination and evaluation of care for patients with complex healthcare needs. You will work autonomously while collaborating closely with the wider multidisciplinary team to ensure patients receive timely and effective interventions. You will undertake comprehensive clinical assessments, develop personalised care plans and coordinate services to meet patient needs. You will be responsible for supporting patients with long-term conditions, frailty and admission avoidance pathways, ensuring care is tailored, proactive and responsive. Alongside your clinical responsibilities, you will provide leadership and support to nursing colleagues within the team. You will help develop staff through supervision, appraisals, mentorship and clinical support, while contributing to service improvement, audit activity and quality initiatives. Key responsibilities include:

Managing a caseload of patients within the Virtual Ward service

Delivering high-quality clinical assessment, planning and evaluation of care

Supporting patients with frailty, long-term conditions and complex healthcare needs

Participating in referral triage and prioritisation of patient needs

Coordinating care with GPs, specialist teams, social care and partner organisations
-
Supporting admission avoidance and early intervention pathways
-
Providing clinical leadership, supervision and support to junior colleagues
-
Contributing to service development, audit and quality improvement initiatives
-
Maintaining accurate and timely clinical records
-
Promoting safe, effective and evidence-based practice at all times
-
Registered Nurse with current NMC registration
-
Minimum of three years post-registration experience
-
Experience working within community and/or acute healthcare settings
-
Experience managing patients with long-term conditions and complex healthcare needs
-
Evidence of continuing professional development
-
Teaching, mentorship or assessing qualification
-
Experience supervising or supporting other staff members
-
Strong clinical assessment and decision-making skills
-
Excellent communication and interpersonal skills
-
Ability to work autonomously and manage a variable workload
-
Good IT skills including Microsoft Office and electronic patient records

Full UK driving licence and access to a vehicle for work purposes

Degree level qualification or working towards one
-
Independent Prescribing qualification, or willingness to undertake

Experience conducting appraisals and clinical supervision

First-line management experience

Knowledge of clinical governance, audit and quality improvement processes

Experience contributing to service development initiatives
