JJob Summary
We are recruiting an experienced Band 6 Clinical Pharmacist to support an NHS Hospital Pharmacy Service.
The role involves autonomous clinical review, medicines optimisation and pharmaceutical care for patients within an acute hospital setting. The post holder must be confident working independently within a fast-paced multidisciplinary environment, managing complex medication regimens and promoting safe, effective, and evidence-based use of medicines.
This is ad-hoc locum work, with forward planning available and regular opportunities to secure shifts in advance.
Key Responsibilities
Autonomous clinical screening and verification of prescriptions and medication orders
Development and implementation of patient-centred pharmaceutical care plans
Management of medicines optimisation for a defined patient group within hospital wards and clinical areas
Provision of clinical pharmacy services, including medication reviews, medicines reconciliation and therapeutic monitoring
Identification, resolution and prevention of medication-related issues and prescribing errors
Provision of medicines information and advice to patients, carers and healthcare professionals
Accurate and timely clinical documentation in line with service standards
Working collaboratively within the multidisciplinary team
Adherence to NHS policies, medicines management procedures and clinical governance standards
Essential Requirements
GPhC-registered Pharmacist
Proven experience within NHS hospital pharmacy services
Experience managing complex medication regimens and providing clinical pharmacy input
Ability to work autonomously and prioritise workload effectively
Strong clinical assessment, communication and documentation skills
Enhanced DBS
Right to work in the UK
Desirable
Experience within acute medicine, surgery, elderly care, critical care or specialist clinical services
Independent Prescriber qualification
Experience carrying out ward-based clinical pharmacy services
Previous NHS Band 6 Clinical Pharmacist experience
