Job Summary
Pathfields Medical Group is looking for a Practice Nurse to join our friendly, forward-thinking, and supportive multidisciplinary team.
This role would particularly suit a Practice Nurse with experience with a strong interest in Learning Disability health reviews and chronic disease management. However, we are equally keen to hear from candidates who are motivated to develop these skills, and we will provide the support and training needed to help you achieve this. We are looking for someone who is passionate about improving outcomes for patients with complex needs and reducing health inequalities within our community.
You will play a key role in delivering high-quality primary care services, supporting patients with long-term conditions and promoting preventative healthcare.

About Us
We are an ambitious and diverse GP Practice Network providing care to over 30,000 patients across six sites in Plymouth.

Key Responsibilities
The Practice Nurse will support the delivery of a range of clinical services including:
- Chronic disease management clinics, including Asthma, COPD, and Diabetes
- Health promotion and preventative healthcare
- Supporting patients with long-term conditions through education and care planning
- Delivering evidence-based nursing care within primary care guidelines
- Working collaboratively with the wider multidisciplinary team
- Maintaining accurate clinical records using NHS clinical systems
- Supporting quality improvement initiatives and patient safety standards
Remote duties, where appropriate, may include chronic disease reviews, patient education, care planning, and virtual consultations.
The Ideal Candidate Will Have
Essential Qualifications
- Registered General Nurse (RGN) with current NMC registration
- Experience working within Primary Care or General Practice
- Knowledge and experience in chronic disease management
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Strong organisational and time management skills
- Commitment to patient-centred care and continuous professional development
Desirable
- Knowledge of EMIS or SystmOne clinical systems
Personal Attributes
· Strong communication and teamwork skills
· Ability to work independently while contributing positively to the wider multidisciplinary team
Salary
Please note that the hourly rate is dependant upon experience.
Working Pattern
18 hours per week
