Full job description

Job Overview
We are seeking a dedicated and compassionate Care Coordinator to join our team. The successful candidate will play a vital role in organising and managing patient care plans, ensuring seamless communication between clients, healthcare professionals, and support services. This position offers an opportunity to make a meaningful difference in the lives of individuals requiring specialised care, while working within a supportive and professional environment. Prior office experience is desirable to facilitate efficient administrative operations.

Duties

Coordinate and monitor individual care plans in collaboration with healthcare providers and clients
Schedule appointments, follow-ups, and support services to meet client needs
Maintain accurate and up-to-date records of client interactions and care activities
Provide occasional hands-on care when necessary.
Act as a point of contact for clients, families, and healthcare professionals to address queries and concerns
Organise documentation, reports, and correspondence related to client care
Assist with administrative tasks such as data entry, filing, and managing schedules using office software
Ensure compliance with organisational policies and confidentiality standards
Support the onboarding of new clients by providing necessary information and guidance

What We’re Looking For

Experience as a Care Coordinator (preferred)
Strong organisational and communication skills
Ability to work under pressure and manage last-minute rota changes
Flexibility to support the service outside standard hours
A caring, patient, and professional approach
Full UK driving licence & access to a car (essential)
