Full job description

We are seeking a compassionate and dedicated Community Support Assistant to provide essential support to individuals in their own homes.

This role involves assisting with daily activities, promoting independence, and ensuring the well-being of clients.

The ideal candidate will possess excellent communication skills, be reliable, and have a genuine desire to make a positive difference in people's lives.

A valid driving licence is a must to facilitate travel between clients' homes.

Duties

Supporting clients with daily routines to promote independence.

Implementing support plans tailored to individual needs, ensuring they are followed accurately.

Providing companionship and emotional support to clients, fostering a friendly environment.

Administering routine medication as prescribed and maintaining accurate records.

Assisting with light household tasks including cleaning, laundry, and simple meal preparation.

Maintaining clear communication with clients, families, and healthcare professionals.

Updating records and document visits efficiently.

Driving between different client locations as required, ensuring timely attendance at appointments.

Requirements

Previous experience in a care home or home care environment is advantageous but not essential, full training will be provided.

Good command of Welsh and English, both spoken and written, for effective communication.

Excellent communication skills to build rapport with clients and team members.

Compassionate attitude with a genuine interest in supporting vulnerable individuals.

Ability to work independently and as part of a team.
