Job Description

The Registered Nurse will provide clinical leadership, nursing oversight, and professional support to individuals receiving care through Rhythmic Care UK Ltd. The postholder will ensure that all care packages requiring nursing input are delivered safely, effectively, and in accordance with current legislation, professional standards, and best practice.

The successful candidate will work collaboratively with multidisciplinary teams, commissioners, GPs, hospitals, community services, families, and care staff to ensure high-quality, person-centred care that promotes independence, dignity, and positive outcomes.

Key Responsibilities

Clinical Leadership

Provide nursing oversight for complex care packages requiring registered nursing input.
Conduct comprehensive clinical assessments and formulate individualised care plans.
Review care plans regularly to ensure they reflect changing clinical needs.
Ensure all nursing interventions are evidence-based and compliant with professional standards.
Monitor service users' physical and mental health and escalate concerns appropriately.
Lead clinical decision-making for deteriorating conditions and emergency situations.

Care Planning and Case Management

Develop, implement, monitor, and evaluate person-centred care plans.
Undertake risk assessments relating to clinical conditions and treatment.
Ensure medication management complies with legal and regulatory requirements.
Coordinate reviews with local authorities, Integrated Care Boards (ICBs), NHS teams, and commissioners.
Support hospital discharge planning and continuity of care.

Governance and Quality Assurance

Maintain compliance with standards set by the UK nursing regulator and relevant healthcare legislation.
Participate in clinical audits and quality improvement initiatives.
Investigate incidents, complaints, safeguarding concerns, and medication errors where required.
Maintain accurate, timely, and confidential clinical records.
Ensure infection prevention and control standards are consistently maintained.

Staff Support and Supervision

Provide clinical guidance and supervision to care workers and senior carers.
Deliver competency assessments and practical observations.
Identify training needs and contribute to workforce development.
Support induction programmes for new staff.
Promote safe moving and handling practices and clinical best practice.

Safeguarding

Recognise and respond appropriately to safeguarding concerns involving adults and children.
Follow safeguarding procedures and reporting requirements.
Work closely with safeguarding authorities and partner agencies where necessary.

Multi-Disciplinary Working

Liaise effectively with:
General Practitioners (GPs)
Community Nurses
Hospitals
Social Workers
Occupational Therapists
Physiotherapists
Speech and Language Therapists
Commissioners
Family members and advocates
Attend multidisciplinary meetings and contribute to clinical reviews.

Professional Responsibilities

Maintain registration and professional standards required for nursing practice in the UK.
Participate in continuing professional development (CPD).
Maintain knowledge of current legislation, guidance, and evidence-based practice.
Work within organisational policies and procedures at all times.

Person Specification

Essential Qualifications

Registered Nurse qualification (Adult, Mental Health, or Learning Disability as appropriate).
Current registration with the relevant UK nursing regulator with no restrictions to practice.
Valid right to work in the United Kingdom.

Essential Experience

Minimum 2 years' post-registration experience.
Experience managing complex care packages in community or residential settings.
Experience undertaking clinical assessments and care planning.
Experience supervising or mentoring healthcare staff.
Experience working with multidisciplinary teams.

Teaching or assessor qualification.
Full UK driving licence and willingness to travel between service locations.

Working Conditions

The role may require:

Community visits and travel between service locations.
Participation in an on-call rota where applicable.
Occasional evening, weekend, or bank holiday working to meet service needs.
Manual handling activities in accordance with training and risk assessments.
