As part of community transformation an exciting role has been developed between TEWV & Tees Valley Primary Care networks (PCN'S).
We are looking for an enthusiastic Nursing Associate to work within the Tees Valley. 50% of the role will include working in the PCN's across the Tees Valley and 50% working into TEWV hub in the One life in Hartlepool.
The role is to ensure that physical health needs of SMI patients are met. The practitioner will be expected to link in with individual GP practices to identify patients outstanding for physical health checks.
An assertive outreach approach will be adopted to engage patients on the SMI register to have their physical health investigations carried out, with an aim to reduce the health inequalities faced.
Time will be shared between TEWV & PCN's meaning the new role will involve time working in our secondary care teams and in primary care settings across the Tees Valley.
You must have the Right to Work in the UK in order to be successfully appointed to this role. Please note, this role does not meet the required eligibility criteria for sponsorship under the Skilled Worker visa and therefore the Trust is unable to offer sponsorship for this particular role.

The successful candidate will be able to work as part of an integrated Community Mental Health Team and Primary Care Services.
Managerial and clinical supervision will be provided to support the successful candidate manage their caseload.
There are varied opportunities for training and development, with clinical development needs being identified and monitored through appraisal.
The successful individual will need a Foundation Degree in Health and Social Care or be a registered Nursing Associate.
