Full job description

Job Summary
We are seeking a dedicated Support Coordinator to join our team. The ideal candidate will have a background of care work and be responsible for matching trained local volunteers with medically vulnerable or lonely clients who have recently been discharged from hospital, or are at risk of hospital admission. This role offers an opportunity to work from home and in Hatfield, Welwyn, St Albans and Hertsmere, with fortnightly visits to the office in Hitchin. We are a small team dedicated to helping our clients feel less lonely and isolated and the Suppoort Coordinator plays a vital part in ensuring smooth operations and delivering excellent service to our clients and a rewarding experience to our volunteers.

The main duties and responsibilities are to:

Volunteers

recruit, interview and process all potential Reach Out volunteers
onboard and deliver training
match and allocate volunteers to individual clients, having regard to their location, availability, experience and compatibility
keep in regular contact with volunteers to ensure they are motivated, safe and effective
ensure volunteers have a positive volunteering experience.

Clients

work in partnership with the Hospital Discharge and Herts Help Community Navigators to identify clients
receive, assess and respond to service requests promptly, courteously and effectively
visit clients in their own homes and complete risk assessments and assess support needs
record sensitive information securely and update as required
carry out follow-up monitoring calls and visit with users of the service.

Admin

undertake administration associated with the project e.g. emails, phone calls and record keeping on the CRM and spreadsheets
understand and comply with the requirements of the Data Protection Act ensuring confidentiality of information in respect of records maintained and tasks undertaken
Support the project manager to create monthly statistical data and produce quarterly monitoring reports
be responsible and accountable for the delivery of a high-quality service and promote good practice
undertake other work that may arise on a day-to-day basis to support the objectives of NH&S CVS, as required by the line manager.

Partners

maintain effective working partnerships with public sector agencies, other voluntary organisations and local businesses. And where appropriate, establish new links
help raise the public awareness of the project together with the Project Manager
represent Reach Out and North Herts & Stevenage CVS at events and meetings as required.

Other

undertake training as required
Support the Project Manager with the development of the Reach Out project

The post holder must hold a full, clean driving license and provide and insure their own vehicle for work. The role is subject to an Enhanced DBS check.

PERSON SPECIFICATION

Qualifications

Essential: GCSE or equivalent in Maths and English

Desirable: Care Certificate NVQ or equivalent in Health & Social Care

Skills/Abilities

Excellent verbal and written communication skills.

Ability to form strong positive relationships with people across a diverse range of backgrounds.

Ability to empower others and encourage participation.

Ability to plan and prioritize workload and meet deadlines.

Excellent IT Skills: Outlook, Word, Excel and data entry on CRM.

Ability to work in a team and individually.

Knowledge

Essential:

Understanding the issues and health needs affecting vulnerable and/or older people.

Understanding of the value of volunteers and a commitment to maximize their potential.

Understanding the need and sensitivity of professional confidentiality.

Desirable:

Good local knowledge of Hertfordshire Services.

Understanding of the voluntary sector.

Experience

Essential:

A background of care work either in the community or in a care home setting.

Experience of safeguarding policies and procedures.

Experience of working with volunteers.

Ability to establish and maintain effective relationships with a wide variety of people.

Desirable:

Experience of working with convalescing, elderly or isolated people.

Experience of recruiting volunteers.

Aptitude

Enthusiastic supporter of community activities, volunteering and independence for vulnerable adults.

Committed to social inclusion.

A friendly, approachable listener.

Able to work occasionally in the evening or at weekends.
