Headway HELP Discharge and Community Support Advisor

Location: Addenbrooke’s Hospital, Cambridge + Community Visits
Part-Time – 22.5 Hours per Week

Salary: £34,000–£35,000 FTE (depending on experience)

About Us

Headway Cambridge & Peterborough is a specialist brain injury charity supporting adults affected by acquired brain injury and stroke, along with their families and carers. We provide person-centred support to help people rebuild confidence, independence, and quality of life following life-changing injuries.

About the Role

We are looking for a compassionate and proactive individual to join our Headway H.E.L.P team based at Addenbrooke’s Hospital.

This rewarding role focuses on supporting patients and families during some of the most difficult stages of their journey following a serious brain injury or stroke — from hospital rehabilitation and discharge through to community support.

You will work closely with hospital teams, rehabilitation providers, social care, and community services to help individuals access the support, advice, and services they need.

Key Responsibilities

Provide emotional and practical support to patients and families during the later stages of the patient journey, supporting transitions from Addenbrooke’s Hospital to rehabilitation services, repatriation closer to home, or discharge into the community, ensuring continuity of support throughout recovery and beyond.
Provide advice, signposting, advocacy, and practical support
Liaise with NHS teams, social care, rehabilitation providers, and community services
Help patients access benefits, care, therapies, equipment, and support services
Promote the Headway H.E.L.P service across the hospital and wider community
Maintain accurate records and support person-centred care planning

About You

We are looking for someone who is:
✔ Experienced in health, social care, rehabilitation, or community support
✔ Confident working with vulnerable adults and complex situations
✔ Compassionate, organised, and an excellent communicator
✔ Experienced in discharge planning, advocacy, or community signposting
✔ Able to work independently and as part of a multidisciplinary team

Experience supporting individuals with brain injury is desirable but not essential — full training will be provided.

Essential Requirements

Degree-level qualification
Minimum 3 years’ experience in a health, social care, or community-based role
Experience supporting vulnerable adults
Strong understanding of discharge planning and referral pathways
