An exciting opportunity has arisen for an enthusiastic and innovative Physiotherapist to work within the Falkirk Health and Social Care Partnerships ReACH Service. The ReACH Teams provide multidisciplinary assessment and rehabilitation for adults in the community.
The role encompasses Physiotherapy assessment, diagnosis and treatment of patients within the community as part of a multidisciplinary team. There will be a focus on preventing unnecessary hospital admission and supporting earlier hospital discharge in line with the national Discharge without Delay programme.
You should have broad previous experience with highly developed skills in community rehabilitation, or frailty and falls prevention. The candidate should have proven record of service development.
As a senior clinician, you will have responsibility for delivering highly skilled physiotherapy to patients in the community and will be a proactive member of the multidisciplinary team.
You will act as a senior clinical decision maker, leading complex care and delivering rehabilitation to ensure patients are optimally rehabilitated effectively and efficiently. You will make onward referral to other services as required.
You will also support the development of less experienced, and unregistered staff within the team. In turn you will be supported to develop into the role and responsibilities of a highly specialist Band 7 clinician in community rehabilitation. Contribution to practice education and service improvement is also expected.
The ReACH Services offers a varied clinical caseload, excellent peer support and training opportunities.
The successful candidate will be based in Falkirk Community Hospital with the ReACH Falkirk Team and would be expected to act as a Forth Valley wide resource for advice on treatment and management of complex patients in conjunction with other Highly Specialist AHP’s in the ReACH Teams.
The ReACH team provides a 7 day service and the post will involve weekend working.

As from 1/4/26, the Agenda for Change full-time working week will be reducing from 37 to 36 hours per week. Part time hours will be reduced pro-rata. However, there will be a corresponding increase in the hourly rate so that pay will not be affected.
