An exciting opportunity for a secondment/ fixed post for a kind and experienced Senior Community Practitioner to join our friendly and dedicated colleagues within the MHSOP Scarborough Community Mental Health Team, 30 hours (reduced hours will be considered).
This opportunity will allow the right person to gain experience working and supporting individuals in the community and support your continuing development within your career.
You will work alongside all members of the multi-disciplinary team. With access to and support from specialist services available in the trust, and excellent working relationships with local partner organisations.
The Practitioners are an integral part of the community pathway as well as working within the proposed 10-year plan with clear direction and goals of increasing care within the community and as a Practitioner they play a significant role in providing preventative measures and interventions working closely in a joined-up approach with our external health colleagues this will continually support reducing hospital admissions both to medical and mental health inpatient units.

As a Band 6 Senior Community Mental Health Practitioner, you are an essential clinical role within the Community Mental Health Team. Band 6 staff are able to deliver the necessary enhanced and effective interventions to reduce the likelihood of acute and psychiatric admissions. They oversee the recovery journey of complex individuals requiring structured evidence-based interventions to enable a good quality of life within the community. They work closely with families and carers to ensure they also have the support available to avoid and reduce carer's stress.
As a Senior Practitioner you will play a significant role in providing preventative measures and interventions, working closely in a joined-up approach with our external partners, to achieve the best possible outcomes and quality of life for people. Senior Practitioners conduct comprehensive biopsychosocial assessments to formulate the needs and risks of patients who referred to the service.
The Practitioners help deliver and oversee pharmacological and non-pharmacological interventions, providing clinical supervision to CSW's. They provide clinically relevant updates via MDT and supervision, accessing wider services and support according to changing patient need. You will also be part of the duty worker rotation within the team
