We have an exciting opportunity for two enthusiastic and motivated dietitians to join the dietetic team at Sheffield Teaching Hospitals NHS Foundation Trust. This role will work within the Sheffield Community and provide a service to the Sheffield Community. This post will also involve supporting our Community Dietetics Improvement Programme.
The STH dietetic department consists of around 65 Dietitians, plus dietetic assistants, and admin staff across 3 hospital sites and Sheffield community. We are a friendly, supportive, and innovative department with a commitment to patient-centered care and best practice.
The successful candidate will be able to demonstrate strong clinical skills, and excellent organisational and interpersonal skills.
To apply for the post you will need to be an HCPC registered Dietitian and ideally have some experience of working within the Community setting.
Community Dietitians work in a variety of settings to provide a citywide service for the residents of Sheffield. The aim of the service is to promote health and prevent disease amongst the population by improving nutrition and raising awareness of the link between nutrition and health. The service works in partnership with Primary and Secondary Care as a key link in care pathways for preventing and managing heart disease, stroke, diabetes, gastrointestinal conditions and malnutrition. We support patients in making lifestyle changes to manage their long-term conditions.
Determine the appropriate dietetic treatment and develop effective management plans for patients within the community setting with a variety of conditions and dietetic diagnoses (e.g malnutrition, gastrointestinal conditions, diabetes etc)
Dietetic assessment and calculation of dietetic requirements of referred patients considering medical, biochemical, social, cultural, and psychological factors.
Provide nutrition and dietary advice suitable to the patient’s medical, biochemical, social and cultural factors which is based on current evidence, incorporating local professional and national guidelines and standards.
Negotiate with patients to enable them to achieve targets overcoming barriers to change including dietary, environmental, and social factors.
Monitor and review dietetic care plans against outcomes. Manage and adjust care plans to facilitate achievement of dietetic goals.
Ensure dietetic interventions can be implemented through continued collaborative interdisciplinary working in and outside the Trust by attending and participating in relevant community multidisciplinary meetings, best interest meetings etc and case conferences. This involves close working relationships with our community colleagues e.g GPs, Community SLT.
Provide dietetic support in a variety of settings e.g in patient’s own homes, face-to-face in clinics, telephone clinics and also in virtual clinics.

There is a full time permanent post & the full time fixed term/secondment post on this vacancy is advertised on a fixed term contract basis for the maximum period until December 2026. Where there is potential, the secondment/fixed term contract may finish early, or in the first instance, the secondment/fixed term contract may be extended due to the internal/external secondment of the substantive post holder. The fixed term contract status of the post holder will be reviewed in accordance with the Trust's legal obligations regarding fixed term contracts and secondments in relation to the substantive post holder.
