Transitions Team-
Join Doncaster’s Transitions Team and support young people with additional needs to achieve meaningful and lasting adult outcomes. Guided by Doncaster’s values of working collaboratively, putting people and families at the centre, and striving for the best possible outcomes, you’ll work alongside partners to deliver high‑quality, strengths‑based support.
Your role will focus on the four pillars of Preparation for Adulthood – employment and education, independent living, good health, and friends, relationships and community inclusion – ensuring plans are purposeful, aspirational and led by what matters most to young people.
This is also an exciting opportunity to help shape and develop transition services, influencing how we respond to local needs and improve outcomes for future generations.
