About the Role
As a Social Worker within the Locality Team, you will be responsible for:

Conducting assessments of individual needs and ensuring appropriate support is provided.
- Working collaboratively with individuals, their families, and other professionals to develop and implement care plans.
- Providing advice, guidance, and support to individuals to promote their independence and wellbeing.
- Ensuring the safeguarding of vulnerable individuals by identifying and addressing risks.
- Maintaining accurate and timely records of interventions and progress.
- Attending and contributing to multi-disciplinary meetings and reviews.
You will be part of a dedicated team of professionals, working together to deliver exceptional care and support to the local community.
About You
The ideal candidate will possess the following skills and attributes:

Qualified Social Worker with appropriate registration (e.g. HCPC).
- Experience working in an adult social care setting, ideally within a Local Authority.
- Sound knowledge of relevant legislation, regulations, and best practices.
- Excellent assessment and intervention skills.
- Strong communication and interpersonal skills, with the ability to build positive relationships.
- Ability to work effectively both independently and as part of a team.
- Good organisational skills, with the ability to manage a diverse caseload.
- Commitment to continuous professional development.
