We are looking for dedicated Social Workers to join our Adult Social Care teams, working with the preparing for adulthood team, supporting people to transition into adult social care, with a focus on developing independence skills and enablement. You'll undertake meaningful, strengths‑based work that enables people to achieve positive outcomes, stay safe, and maintain their wellbeing.

About the Role

As a Social Worker in our Health and Adult Service’s, you will:

Undertake asset‑ and strength‑based assessments with individuals from the age of 14 to 25 and carers.
Determine eligibility under the Care Act and clearly communicate outcomes.
Work creatively with individuals to develop personalised care and support plans that represent best value.
Provide professional information, advice and guidance, exploring universal services, voluntary sector options, and alternative funding streams.
Support the uptake of direct payments and initiatives such as reablement, personalisation, and self‑directed support.
Undertake reviews, reassessments, and emergency interventions as required.
Lead safeguarding adults’ information‑gathering and investigations (following training and experience).
Maintain high‑quality, timely case records in line with council standards.
Work collaboratively with partners across health, community and voluntary sectors.
Participate in the duty system and support performance data collection.
This is a countywide role, therefore the ability to travel across the county is essential.
This role will include opportunities to work alongside specialist teams, including Mental Health, CYPS, CHC, DoLS/Review, and Education, supporting your professional growth.
