Exciting Opportunity for a Passionate Practice Nurse.

Key Responsibilities:

Conduct assessments, screenings, and chronic disease management.
Deliver immunisations, wound care, and health education.
Collaborate with the GPs and clinical team to develop patient treatment plans.
Contribute to a culture of proactive, preventative healthcare.

About You:

Registered with the NMC, experience in a similar role is desirable but not essential.
Skilled in-patient consultations, with a compassionate approach.
Strong team player who is adaptable and proactive.
Qualifications in chronic disease management or another specialty are a bonus!
