Job Overview

Leicestershire Partnership NHS Trust are working together with Adult Social Care across Leicester, Leicestershire and Rutland (LLR) in order to deliver our HomeFirst service offer alongside core Community Therapy.

We have had significant investment in staffing in order to ensure that services can be delivered by the right therapist in the right place at the right time, and to ensure that the local population across LLR receive timely high-quality therapy.

This is therefore a really exciting time to be joining the service which brings together an integrated health and social care offer delivered by community-based nursing, therapy and adult social care teams and will allow you to use your therapy skills to:

Support people to remain in their homes when they are having a health or social care crisis, rather than needing to go into hospital or a care home, help people get home from hospital quickly and provide them with rehabilitation and reablement to help restore their health, wellbeing and independence.

Main duties of the job

We are looking for Band 6 Physiotherapists to provide an effective and efficient therapy service. This post requires a level of personal responsibility for someone highly motivated, resilient and proactive with the ability to prioritise and manage their own allocated caseload, use their initiative and the ability to work autonomously and as part of a team. Demonstrate excellent communication, organisational skills and be able to adapt to changing and pressurised environment. Working with Occupational Therapists and Technical Instructors to carryout comprehensive functional assessments and interventions to help patients live as independently as possible.

As part of your role you will have the opportunity to work within the Discharge Hub to represent Leicestershire Partnership Trust completing triage at the point of referral alongside partners in Adult Social Care and University Hospitals of Leicester. Together you will be part of the decision-making team to support flow and discharges from our hospitals following the Discharge to Assess principles.

The therapy teams operate in an integrated way whereby Therapists are encouraged to work synergistically in order to provide the best possible outcomes for patients. You will be encouraged and supported to grow your skillset, and to challenge the way we do things. We are looking for determined and self-motivated individuals who can make a difference to the care of patients and to promote therapy services within the trust.

Job Description Summary (Main duties and responsibilities)

3. Delivery of Effective Healthcare within the organisation
3.1. To be responsible for own practice and accountable for all aspects of the Physiotherapy assessment and treatment (information gathering, assessment, treatment planning, intervention, evaluation and discharge planning) as appropriate working with a person centred approach.
3.2. To assist team/clinical/operational leaders in the organisation and co-ordination of the service provided.
3.3. To manage own time constructively and prioritise clinical workload working with patients on a 1:1 basis or in a group.
3.4. To work in partnership with team leads, managers and other practitioners in the effective management of referrals, case prioritisation and care co-ordination within clinical care pathways.
3.5. To work effectively with members of the Multi-Disciplinary Team and other related agencies to ensure a needs-led service is maintained.
3.6. To attend and or lead clinical case reviews and present appropriate Physiotherapy feedback accordingly.
3.7. To maintain accurate and timely patient records in the electronic patient record and to fulfil record keeping standards in accordance with Leicestershire Partnership NHS Trust policy, CSP and HCPC.
3.8. To work flexibly and in partnership with the multidisciplinary team, to provide a holistic approach in meeting the needs of patients and the service.
3.9 To work within an advisory capacity as appropriate within the post and skills/experience of post holder in order to inform others (services, family, carers, agencies) regarding patient needs.
3.10 To delegate workload effectively to other team members.
3.11 To deliver supervision to Band 5 or unregistered staff as required.
3.12 To support and deliver training in relation to patient needs.
3.13 To participate in and lead where appropriate in-service training within Physiotherapy and the wider service.
4. Establishing Effective Leadership and Communication
4.1. To communicate and work collaboratively with patients, families/carers, other disciplines and other agencies to ensure good professional working relationships are maintained.
4.2 To have clinical responsibility for a designated caseload of patients and to organise this effectively with regard to clinical priorities and in line with the Trusts waiting/response times and service priorities.

4.3 To communicate effectively and work collaboratively with patients and families/carers to maximise an individual’s wellbeing. Communication skills of explanation, motivation, negotiation and sensitivity will be used in a variety of situations.
4.4. To participate in all relevant clinical team and professional meetings to contribute to the optimum level of care for patients and ensure effective liaison with other agencies, families/carers and to contribute to effective team working and development.
4.5 To provide specialist advice, teaching and education to patients, families/carers and other professionals to promote understanding of the aims of Physiotherapy interventions and to ensure a consistent approach to patient care.
4.6. To provide support to the management of the therapy team as delegated by and agreed with the Team/Clinical/Operational Lead and to be responsible for the supervisory management of relevant clinical staff including the implementation and monitoring of appraisals and identification of continuous professional development.
4.7. To keep other members of the Multi-Disciplinary Team aware of developments within the therapy service.
5. Continuing Education, Professional and Personal Development
5.1. Undertake the Trust’s corporate and local induction, and maintain your learning and compliance with training requirements for your role.
5.2. Participate in supervision via agreed review and appraisal mechanisms.
5.3. It is mandatory for all professionally qualified staff and clinical support staff to actively participate in clinical/managerial/safeguarding supervision as an integral part of their professional development.
5.4. To initiate new ideas and modern working practices for development within the Service and to improve the quality standards of care through the use of evidence based practice and outcomes for all patients.
5.5. To adhere to the Chartered Society of Physiotherapy’s Code of Conduct and HCPC standards of proficiency as well as local Trust standards and policies to make autonomous clinical decisions and participate in the development of Physiotherapy quality standards, local clinical guidelines and adhere to trust policies and procedures.
5.6. To participate when appropriate in training to external agencies in consultation with Team/Clinical/Operational Lead.
5.7. To take responsibility for the clinical and management supervision of other Physiotherapy staff, Band 5 or unregistered staff as appropriately agreed with Team/Clinical/Operational Lead.

5.8. To ensure that supervision contracts are negotiated with all Physiotherapy staff responsible for.
5.9 To accept students on placement, using a variety of placement models, and supports their development and provides experience in the clinical field.
5.10 To maintain own clinical and professional development by keep abreast of current national and local health and social care priorities. To keep up to date with treatment techniques and interventions through training and other activities in order to ensure maintenance of competencies, use of relevant evidence based practice into treatment and continual professional development. This will be done in consultation with the Team/Clinical/Operational Lead and Professional Leads.
5.11 To participate fully in postgraduate studies relevant to the area of practice and complete a record of all learning experiences as part of CPD in line with HCPC registration requirements.
5.12 To utilise online appraisal as a tool for identifying strengths, areas of interest and needs in conjunction with Team/Clinical/Operational Lead.
5.13. To be an active member of in-service training programme including critiquing and implementation of recent evidence, attend and present at peer group meeting, training sessions, external courses etc. and carry out reflective practice.
6. Clinical Governance, Reduction of Risk, Audit and Research
6.1 To assist in the formulation of Physiotherapy and MDT standards, operational policies and service documentation.
6.2 To contribute to and take a lead in audit, quality improvement, research and development and recommendations on clinical practice.
6.3. To comply with the policies and procedures relating to the service in accordance with Trust Directives.
6.4. To be aware of national and local policies and directives affecting provision of clinical services.
6.5. To introduce the appropriate quality measures in conjunction with Team /Clinical/Operational Lead to ensure service activity is monitored more effectively.
6.6. To keep other members of the Multi-Disciplinary Team aware of developments within the therapy Service.
6.7. Demonstrate understanding of clinical governance and risk management and participate in initiatives as appropriate with support from Team/Clinical/Operational Lead.
7. Management and use of Resources and Information
7.1. All records that the role is responsible for or modifies must be kept up to date and maintained in an accurate and diligent manner.

7.2. Be responsible for the security and maintenance of service equipment and resources, ensuring that standards of safety and infection control are maintained.
7.3. Demonstrates compliance with the Medical Devices Policy.
7.4. To ensure confidentiality of patients and employers business at all times.
7.5 To maintain high standards of Clinical Record Keeping, report writing including electronic data and recording e.g., ESR, and provide statistical returns as requested.
7.6. To contribute to compilation of reports and policy documents when necessary.
7.7. To provide statistical information to contribute to the evaluation and future planning of the service.
7.8. Manage resources in line with CQC, LPT and CSP standards.
