You'll need to be a car driver with your own vehicle to visit patients in their own homes. We offer a generous mileage allowance to cover the use of your car for NHS business, which includes both mileage and depreciation.

Main duties, tasks & skills required
As a community nurse, you'll provide care to patients in their homes, managing a range of complex health issues and collaborating with various other disciplines.
You'll also work closely with the Hospital @ Home team, focused on preventing acute admission. You'll develop a broad range of nursing skills, including IV medication administration, palliative and end-of-life care, complex wound management, catheter and continence care, diabetes management, PICC line care, medication administration, and drain tube management.
You'll need to make autonomous clinical decisions and create holistic care plans, with the support of your colleagues and Team Leaders. Comprehensive training and guidance will be provided by our dedicated Clinical Lead Nurses.

Community Nurses work as part of a team providing high quality and compassionate care to individuals. The Community Nurse is an autonomous practitioner who is responsible for the holistic assessment of both complex and simple care needs, the development, implementation and evaluation of care and delegation of care where appropriate to other members of the team. They will support the multidisciplinary team in the delivery of high-quality care to enable individuals to achieve their person-centred goals.
The Community Nurse will take responsibility of the patients care pathway and non-clinical duties in the absence of the Team Leader. They will have the understanding to address complex problems in their day-to-day work, reporting concerns to a Team Leader.
The Community Nurse will be able to work effectively as part of a team and will always act within the limits of their competence, knowing who to ask for help and support as required.
