We have an exciting opportunity for a motivated and forward-thinking Clinical Lead Nurse to join our highly valued Care Home Intensive Support Team (CHIST) .

CHIST is a dynamic, responsive service that brings together Nursing, OT, and Speech and Language Therapy professionals to provide targeted, specialist support to care homes across Staffordshire. Our mission is simple: to improve health outcomes, enhance quality of care, and help residents live healthier, more fulfilling lives while avoiding unnecessary hospital admissions.

As a Clinical Lead Nurse, you'll be at the forefront of supporting care homes through hands-on clinical interventions, expert advice, and collaborative working. You'll play a key role in helping care home teams deliver outstanding care while supporting residents' health, wellbeing, and independence.

You'll collaborate with colleagues and partners both in person and through Microsoft Teams, so you'll be comfortable working in a hybrid environment and maintaining strong professional relationships across services.

Deliver practical clinical advice, guidance and education to care home staff.
Provide specialist nursing expertise tailored to residents' individual needs.
Support the development of personalised care plans that achieve the best possible outcomes.
Work closely with health and social care partners to ensure coordinated, seamless care.
Identify risks early, prevent deterioration and help reduce avoidable hospital admissions.
Promote high standards of care through evidence-based practice and clinical leadership.

What We're Looking For

We're seeking an enthusiastic, compassionate nurse who enjoys building relationships, supporting others and driving quality improvement.

You'll have:
Strong clinical nursing skills and experience.
The ability to build positive, productive relationships with care home teams and partner organisations.
Confidence in providing clinical leadership and managing complex situations.
Excellent communication and interpersonal skills.
A proactive, flexible approach to problem solving.
A genuine passion for improving the lives of care home residents and their families.
