Full job description

Overview

We are seeking a dedicated and compassionate Care Co-ordinator to join our team, responsible for overseeing and organising care services within a care home specialising in senior care. The ideal candidate will possess strong leadership abilities, excellent communication skills, and experience in supervising care staff. This role offers an opportunity to make a meaningful difference in the lives of residents while ensuring the delivery of high-quality personalised care. The Care Co-ordinator will be instrumental in developing and implementing care plans, supervising staff, and maintaining effective communication with residents, families, and healthcare professionals.

Responsibilities

Coordinate and oversee personalised care plans, ensuring they are accurate, up to date, and reflect each individual's needs and goals.
Work collaboratively with families, social workers, healthcare professionals, and multidisciplinary teams.
Support the planning and delivery of safe, effective respite stays.
Complete assessments, risk assessments, reviews, and care documentation to a high standard.
Ensure compliance with relevant legislation, safeguarding procedures, and regulatory standards.
Monitor care quality and identify opportunities for continuous improvement.
Support and mentor care staff, promoting best practice and person-centred approaches.
Participate in admissions, transitions, and discharge planning.
Assist with audits, quality assurance, and service development.

Maintain accurate electronic records and contribute to reporting requirements

We're looking for someone who has:

Experience working with individuals with autism, learning disabilities, and complex health needs.
Previous experience in a care coordination, senior support worker, or supervisory role within health or social care.
Excellent organisational and communication skills.
A sound understanding of person-centred care, safeguarding, positive behaviour support, and risk management.
The ability to build positive relationships with families, professionals, and colleagues.
Confidence using electronic care management systems and maintaining accurate records.
A flexible, positive, and solution-focused approach.

Essential Requirements

A NVQ Level 3 in Health and Social Care (or equivalent) as a minimum.
Willingness to work towards Level 5 Leadership and Management if not already achieved.
Knowledge of relevant legislation and care standards.
Enhanced DBS check (required).
Right to work in the UK.
