What you’ll do:
Complete holistic assessments and create care plans that meet individual needs
Act as a care coordinator, supporting smooth transitions and effective communication
Deliver a range of clinical interventions such as wound care, IV therapies and end‑of‑life support
Monitor caseloads proactively and respond to changes in patients’ conditions
Maintain accurate, up‑to‑date electronic care records
What we’re looking for:
A registered nurse with up‑to‑date clinical knowledge and evidence of ongoing development
Experience of community nursing or caring for people with long‑term conditions
Strong communication skills and confidence working with patients, families and partners
Ability to work independently, prioritise work and support colleagues through supervision
Willingness to work towards non‑medical prescribing if not already achieved
