Outreach Worker Responsibilities for 18+ Young Person

1. Home-Based Support

Conduct regular home visits to provide one-to-one support and guidance.
Develop positive, trusting relationships with the young person.
Monitor the young person’s well-being, safety, and mental health.
Assist with daily living skills such as cooking, cleaning, and household management.
Support the young person in establishing routines and maintaining a structured lifestyle.

2. Independent Living Skills

Support budgeting, financial management, and paying bills.
Help with meal planning, grocery shopping, and nutrition.
Provide guidance on personal care, hygiene, and health routines.
Support education, training, or employment goals.
Encourage skill development for living independently (e.g., using public transport, time management, problem-solving).

3. Community Engagement

Accompany the young person to appointments (medical, educational, social, or recreational).
Facilitate access to local services, support groups, and community resources.
Encourage participation in social and recreational activities to reduce isolation.

4. Advocacy and Liaison

Act as a link between the young person and social workers, healthcare providers, or other professionals.
Attend meetings or appointments with the young person where appropriate.
Advocate for the young person’s rights, choices, and preferences.

5. Risk Management and Safeguarding

Identify and respond to safeguarding concerns or risks.
Support the young person in managing challenging behaviors safely.
Follow organizational policies on confidentiality, safeguarding, and health and safety.
Report incidents, concerns, or changes in behavior promptly.

6. Record Keeping and Reporting

Maintain accurate, up-to-date records of home visits and support provided.
Document progress towards goals and outcomes.
Communicate effectively with the team and management regarding any issues or successes.

7. Emotional and Social Support

Provide emotional support and encouragement.
Help the young person develop coping strategies and resilience.
Support the young person to build and maintain positive relationships.

8. Goal Planning and Personal Development

Assist in creating and reviewing personal development plans.
Support the young person to set realistic goals and achieve milestones.
Encourage self-confidence and independence in decision-making.

Pay: £12.71 per hour
