Full job description

Location; Aylesbury
Salary: £ 38,000.00 to 42000.00 per annum fulltime basis (dependent on experience)
Hours: Part-time, 20-25 hours per week

We are seeking a compassionate, motivated, Care Manager – who is a registered nurse (part time) to support the delivery of high-quality, person-centred nursing and care services for individuals living in their own homes.

Working alongside the Registered Manager, you will play a key role in ensuring our service users receive safe, effective, and compassionate care that promotes independence, dignity, and choice. This is an excellent opportunity for a registered nurse who is passionate about community-based care and leadership.

What you will do;

Support the Registered Nurse Manager in the day-to-day management of the homecare service.
Support care staff with Induction, training(shared), regular supervisions and appraisal of care staff.
Conduct nursing assessments, care planning, and regular reviews to ensure care remains responsive to individual needs.
Provide professional nursing care including medication management, wound care, catheter care, and other clinical tasks when needed.
Monitor and maintain high standards of clinical practice, governance, and quality assurance.
Undertake spot checks, competency assessments, and clinical audits.
Ensure compliance with relevant legislation, professional standards, and organisational policies.
Conduct complaint and safeguarding investigations and incident management where required.
Work closely and build positive relationships with service users, families, healthcare professionals, and external agencies.
Supervise operations staff of rota planning and provide support in sourcing new care staff.
No accounts or payroll related responsibility.

We are looking for someone who has:

Current registration with the Nursing and Midwifery Council (NMC).
Experience working within community nursing, homecare, or domiciliary care services.
Previous leadership or supervisory experience.
Strong communication, organisational, and problem-solving abilities.
A thorough understanding of safeguarding, infection prevention, medication management, and clinical governance.
A commitment to delivering person-centred care within people's own homes.
A full UK driving licence and access to a vehicle (essential).

Desirable

Qualification in leadership or management.
Experience preparing for regulatory inspections.
Knowledge of domiciliary care regulations and quality standards.
Experience supporting individuals with complex health needs, long-term conditions, or palliative care.
