What Can You Expect?
You will be joining us in the role of Patient Discharge Coordinator, based at the Royal Cornwall Hospital and occasionally our base in Redruth. This is a full-time, permanent position (37.5 hours per week, Monday to Friday).
As the Patient Discharge Coordinator, you will be responsible for ensuring the smooth and timely discharge of patients from hospital or the completion of treatment by proactively liaising with all key stakeholders. This will include working closely with patients, healthcare professionals, and HTG staff to coordinate and deliver efficient, effective, and patient-centred transport solutions that support safe and timely transitions of care.
