Full job description

The post holder is accountable to the Operational Services Manager, and will be clinically supervised by area Team Manager, but is required to undertake the duties of the post without direct supervision. The post holder will offer assessment and support through the diagnostic journey of the individual and their carers/family, across the care environment. The post holder will offer advice, support and signposting to individuals and their carers and work very closely with the other teams in the Older Adult Community Mental Health, Dementia & Frailty service, integrated community services, primary care, third sector organisations and others involved in the individual’s care and carer support. The post holder will ensure that an integrated service is provided to the patient, liaising with other services as required, throughout the diagnostic pathway. Patients, relatives, carers and all staff will find the post holder flexible, approachable, helpful and co-operative, upholding the ethos of EPUT of good customer care at all times with an awareness of own attitude at all times.

1. The post holder will be expected to act as an allocated senior nurse, undertaking Open Compassionate Empowering assessments and supporting individuals and their carers through the dementia diagnostic pathways.

2. The post holder will have close links in with the other teams in the Older Adult Community Mental Health, Dementia & Frailty Service.

3. The post holder will facilitate diagnosis by assessment and use off the appropriate individualised pathway.

4. The post holder will be required to fast track to dementia diagnosis, via suitable qualification and support, as identified

5. The post holder will, if appropriately qualified, offer expert and appropriate prescribing advice on the treatment of dementia and related psychological and behavioural aspects, and to act as an independent prescriber in the role, supported by the appropriate qualification.

6. Ensure effective lines of communication and good networking exists, both with EPUT and with other Community, Primary and Social Care Services.

7. Disseminate specialist knowledge across the range of work areas and practices underpinned by sound theoretical knowledge.

8. To participate when required with local MDT’s.

1. Responsible for monitoring the quality of the service within their catchment area and participate in agreed clinical quality assurance activities, leading on clinical audits when required, practice development and innovation to improve services and apply evidence based practice.

4. Act as role model at all times, able to demonstrate excellent leadership skills

5. Provide and ensure the availability of specialist advice, support and consultation, to staff within the Primary Healthcare Team, Care Home Teams, outside Teams and other agencies as required.

6. As required undertake appraisals and ensure the training needs of staff are met via the appraisal system and also adopt a cascaded system of appraisal, to ensure all staff needs are met.

7 Establish and maintain a professional and effective working relationships with colleagues.
