Case Manager Assistant (Self-Employed)

Job Description

To provide self-employed administrative and practical support to the Case Manager in coordinating rehabilitation, care, and support services for clients with complex needs. The role includes assisting with case coordination, maintaining records, arranging appointments, liaising with professionals and families, and supporting the delivery of client rehabilitation plans.

Key Responsibilities

Provide administrative and coordination support to the Case Manager.
Maintain accurate and confidential client records and case notes.
Arrange and coordinate appointments, meetings, therapy sessions, and reviews.
Liaise with clients, families, therapists, support workers, solicitors, insurers, schools, and healthcare professionals.
Assist with sourcing support workers, therapies, equipment, and services.
Monitor ongoing support arrangements and report concerns or changes to the Case Manager.
Prepare reports, correspondence, meeting notes, and documentation as required.
Assist with tracking invoices, budgets, and authorised expenditure.
Attend client visits and multidisciplinary meetings where appropriate.
Support implementation of rehabilitation and support plans under the direction of the Case Manager.
Ensure compliance with GDPR, confidentiality requirements, safeguarding procedures, and company policies.
Work independently and manage workload effectively within agreed deadlines.

Person Specification

Essential

Previous experience in administration, health, social care, rehabilitation, or support coordination.
Excellent organisational and communication skills.
Ability to work independently and manage competing priorities.
Professional, reliable, and flexible approach.
Good IT skills including Microsoft Office.
Ability to maintain confidentiality and professional boundaries.
Full UK driving licence and access to own vehicle.

Desirable

Experience working with clients with brain injury, spinal injury, or complex disabilities.
Knowledge of rehabilitation or personal injury case management.
Experience coordinating care or therapy services.
Understanding of safeguarding and GDPR requirements.

Terms of Engagement

Self-employed contractor role.
Responsible for own tax, National Insurance, and insurance arrangements.
Flexible hours based on client and business needs.
Hybrid working including remote work, community visits, and meetings.
Mileage and agreed expenses reimbursed where applicable.

Rate of Pay

Hourly rate dependent on experience and qualifications.

Work Location: Hybrid remote in Liverpool
