Intensive Support Navigator

About the role
The role of the navigator will be to provide an intense support package to those referred into the Intensive Support service within Birmingham.
Referrals will be single people who may present as challenging and are likely to have multiple and complex needs.
Navigators will develop a trauma informed support journey for each customer focussing on skills and resilience, Life-skills, health & wellbeing, mental health engagement, social/community engagement, social wellbeing and digital inclusio
Across the three partner providers additional specialisms will be fostered:
CGL Harm Reduction, Substance Misuse, Social prescribing, and treatment support
St Basils under 25’s, care leavers, complex barriers and mental health support
Trident over 25’s, complex barriers, and mental health support
What you’ll be doing
- Build relationships with and support individuals to access the support they need.
- Be both PIE and Trauma Informed
- Hold a caseload of people who present with multiple and complex needs
- Develop co-produced, outcome focused support plans that reflect each participant’s personal strengths and goals.
- Work to the principals of current/existing framework supporting participants to exercise choice and control over their lives.
- Co-ordinate interventions provided by other agencies to ensure that the right support is offered at the right time to enable the recipient to progress on their journey to recovery
- Build and maintain networks of support for the individual across Birmingham by acting as the co-ordinator and navigator for interventions provided by other agencies.
- Work with the individual to develop a co-produced and outcome focused support plan
- Lead on the completion of Safety Plans (Risk Assessments) with individuals and professionals around the person to ensure safe practice at work.
- Adopt a step-up, step-down approach to empowering the individual.
- Proactively approach the promotion of traditional values, demonstrating pride in the work and service delivery.
- Attending all training courses and delivering on the outcomes.
- To ensure the health, wellbeing and safety of vulnerable adults, young people and children in line with Safeguarding regulations.
Who we’re looking for?
- Ensure individuals details are entered onto INFORM our case management system accurately and punctually
- Works in accordance with the Service operating model
- Develop, maintain and coordinate effective working relationships with external agencies.
- Possess a robust knowledge and experience of homelessness and the paths that lead to it.
- Excellent communication skills as well as a customer focussed approach are essential
- Ability to work in a target driven environment
- People management experience in leading, mentoring and developing a team by providing on-going support
- You will need to demonstrate your organisational skills and effective administrative skills
