Main Duties of the Job
This advanced clinical role requires a high degree of autonomy, professional judgement, and leadership. You will carry out comprehensive assessments, diagnose and manage conditions, and develop evidence-based, personalised care plans in partnership with patients and multidisciplinary teams.
Working autonomously, you will provide safe, timely clinical decision-making for patients with acute, long-term, and complex needs, applying advanced clinical and prescribing skills where qualified. Beyond direct patient care, you will contribute to service development, quality improvement, and innovative ways of working, collaborating with divisional and national teams to enhance processes and support sustainable service transformation. Flexibility, initiative, and commitment are essential to maintaining high standards of care, governance, and operational delivery.

Job Description

Clinical practice
· Work autonomously as an advanced practitioner within the specialty, managing a caseload of patients delivering individualised direct patient care
· Direct responsibility for assessment, examination, investigation and diagnosis of patients within their area of work.
· Appropriately treat patients, resulting in the safe management and appropriate referral or discharge of patients with undifferentiated and undiagnosed presentations
· Receive referrals via a variety of sources, including direct patient referral. Manage own caseload and clinical priorities according to agreed protocols and working practices
· Participate in multidisciplinary clinics, participate in ward rounds, patient reviews and multidisciplinary team meetings
· Undertake a variety of clinical skills and provide treatment/ advice as per speciality and scope of practice. Using agreed protocols of clinical practice and professional guidelines. This service may be provided within an acute secondary healthcare facility or in primary, community or home care settings
· Within scope of practice and clinical competence request and / or undertake diagnostic procedures and clinical investigations related to plans of care
· Utilise scope of practice to undertake non-medical prescribing role and provide advice on medicine management issues associated with the patient specialty group. Work within Trust policy for Medicines Management
· Utilise advanced knowledge and skills relating to the speciality to provide specialist advice to other members of the multidisciplinary team on the basis of patient assessment

Pre-employment Requirements
Vaccinations
For this role, you will be required to provide evidence of full Hepatitis B vaccination including three doses with titre level and or booster, along with MMR and Varicella vaccination status. Support will be provided where required.

Person Specification
Qualifications and Experience
Essential
Registered first-level general nurse.
Recognised degree qualification in autonomous practice.
Minimum of five years post-registration experience.
Minimum of five years’ recent experience in primary and community nursing.
Mentor or teaching qualification.
Clinical supervision training and experience.
Extended or supplementary prescribing qualification.
Masters in Advanced Practice and be an independent prescriber.
Desirable
MSc or equivalent
Experience of developing and maintaining effective partnerships with external organisations
Knowledge
Essential
Understanding of the local healthcare system, including its priorities and challenges.
Understanding of national health and social care policy and emerging strategies.
Understanding of the evolving Advanced Clinical Practitioner role.
Advanced clinical practice in managing minor illness and acute presentations.

Understanding of clinical governance within Primary Care.
