Full job description

We are looking for a caring, reliable, and enthusiastic Outreach Worker to support a young lady to access and enjoy activities within her local community.

This is a rewarding role where you will help promote independence, confidence, and wellbeing by providing one-to-one support tailored to her interests and goals.

Key Responsibilities

Support the young lady to access community activities, appointments, and social opportunities.
Encourage independence while providing appropriate support.
Assist with planning enjoyable and meaningful outings.
Provide safe, person-centred support at all times.
Maintain accurate records and communicate effectively with family and the wider support team.
Follow care plans and risk assessments.

Essential Requirements

Full UK driving licence and access to a reliable vehicle (essential).
Good knowledge and understanding of epilepsy, including recognising seizures and responding appropriately.
A caring, patient, and positive attitude.
Excellent communication and interpersonal skills.
Reliable, flexible, and able to work independently.
Previous experience supporting people with additional needs or disabilities is desirable.
