We are looking for dedicated Social Workers to join our Adult Social Care teams, working with the preparing for adulthood team, supporting people to transition into adult social care, with a focus on developing independence skills and enablement. You'll undertake meaningful, strengths‑based work that enables people to achieve positive outcomes, stay safe, and maintain their wellbeing.
About the Role
As a Social Worker in our Health and Adult Service’s, you will:
- Undertake asset‑ and strength‑based assessments with individuals from the age of 14 to 25 and carers.
- Determine eligibility under the Care Act and clearly communicate outcomes.
- Work creatively with individuals to develop personalised care and support plans that represent best value.
- Provide professional information, advice and guidance, exploring universal services, voluntary sector options, and alternative funding streams.
- Support the uptake of direct payments and initiatives such as reablement, personalisation, and self‑directed support.
- Undertake reviews, reassessments, and emergency interventions as required.
- Lead safeguarding adults’ information‑gathering and investigations (following training and experience).
- Maintain high‑quality, timely case records in line with council standards.
- Work collaboratively with partners across health, community and voluntary sectors.
- Participate in the duty system and support performance data collection.
- This is a countywide role, therefore the ability to travel across the county is essential.
- This role will include opportunities to work alongside specialist teams, including Mental Health, CYPS, CHC, DoLS/Review, and Education, supporting your professional growth.
