The Role
We are looking for compassionate, reliable and enthusiastic Community Support Workers to support adults with learning disabilities, autism, mental health needs, physical disabilities and age-related support needs.
You will help individuals access meaningful activities within the community, develop independence, build social connections and achieve outcomes identified within their support plans.
Key Responsibilities
- Support individuals to access community activities and local resources.
- Assist people to develop independent living skills.
- Promote social inclusion, confidence and wellbeing.
- Support participation in recreational, occupational, educational and social activities.
- Maintain accurate records and progress notes.
- Follow safeguarding, health and safety and confidentiality procedures.
- Work collaboratively with families, carers and professionals where appropriate.
- Promote dignity, choice, independence and person-centred support at all times.

Requirements
- Caring and compassionate nature
- Excellent communication skills
- Ability to work independently and as part of a team
- Commitment to promoting independence and inclusion
- Enhanced DBS (or willingness to obtain one)
- Right to work in the UK
- Full driving licence desirable but not essential
