An exciting opportunity has arisen to join the Families’ Health and Wellbeing (FHWS) 0-19 Service delivering public health services to children and families across Staffordshire. We have a 30 hours a week permanent vacancy for a Band 5 Community Staff Nurse based in the Cannock & Rugeley team. This is a development post whereby there may be an opportunity for future development/secondment to complete the SCPHN HV/SN course.
The Families’ Health and Wellbeing Service (0-19) in Staffordshire offers an integrated, flexible and family focused service providing high quality, compassionate, personalised support and care to Children and Young People. As a registered nurse, you will be part of a team delivering the 0-19 Healthy Child Programme for a varied, unique and diverse population, in a supportive skill mix team, working closely with our partner agencies across health, social care and the local authority. The successful applicant will need to work to the service specification and demonstrate the trust values at all times. You will be expected to demonstrate knowledge and understanding of all aspects of child development, safeguarding and will carryout some aspects of the mandated contacts of the service model.
Training to achieve necessary competencies will be provided
Assessment of child growth- weigh & measure babies and young people.
Competent in infant feeding & nutrition and starting solid food.
Competent in assessment of child development.
Competent in public health advice delivery - home safety, vaccinations, dental care, safe sleep, behaviour.
Assessment of maternal mental health and referral onwards.
Knowledge and competence in safeguarding and child protection.
Knowledge of enuresis and child continence/constipation.

The Post holder will:
Be competent as appropriate to the requirement of the

You will need to be willing to travel within the Burton and Uttoxeter areas, as the successful candidate will work between both areas.
You will be a Registered Nurse with the NMC in the UK and have a keen interest in working with children.
You will be responsible for a defined population of service users in a variety of settings across the Trust including the community
You will be skilled in the assessment of care and support needs and in the development, implementation and evaluation of care and support packages.
You will have supervisory responsibilities as required (e.g. work allocation) and may have some responsibility for service developments.
You will deliver delegated aspects of the 0-19 Healthy Child Programme, ensuring key objectives and outcomes are met
You will have the ability to use and undertake training regarding electronic based IT platforms including electronic patient records, including Rio and in the future the E- Red Book, Patient Knows
The post may at times require working between several health clinics in a local area as per needs of the service.
The post holder will be part of the 0-19 Families Health and Wellbeing Service providing public health advice, information, guidance, and referral and providing early intervention. In doing so, they will contribute to the assessment, planning, delivery and evaluation of programmes of health, ensuring that they practice within their sphere of competence and knowledge.
They will work to professional codes, standards and guidance at all times ensuring that their practice is grounded in evidence based theoretical and practical knowledge.
The post holder will promote the health of children aged 0-19 within the Staffordshire area through the public health agenda, thus enabling each child to reach his/her development, and educational potential.
This is an exciting opportunity that requires the Staff Nurse to work closely with multi-disciplinary teams in addressing a range of care and support issues across a spectrum of needs and preventing unnecessary hospital admissions. They will contribute to the delivery of the Healthy Child Programme (0-19); ensuring key objectives and outcomes are achieved and to the improvement of the health and wellbeing of the population served. The nature of the service will be determined by local service specifications.
To adhere to and apply the Nursing and Midwifery Council (NMC) Code of Conduct and all other relevant guidance from the, Nursing & Midwifery Council (NMC) and the Trust. To be professionally accountable and responsible for all aspects of own work.
To establish and maintain effective working relationships both within the Partnership Trust and with external organisations in order to meet service requirements and support and demonstrate the Trust’s equality values.
To assess children, young people, families and carers, care and /support needs. Formulates, develop, implement and evaluate programmes of care and support. The care and support plans may include the following:
Promoting independence/self-management and maintaining safety
Increased functional ability
Improvement in health and wellbeing including mental health
Interventions in disease/condition management
Prevention and reducing of health inequalities
Admission avoidance
Early supported discharge
Case management/ treatment/ care plans
Palliative and end of life care
To weigh and measure children, young people and infants and record accurately
To support the public health practitioners with the completion of caseload profiles identifying health needs
Health promotion, advice and guidance
Signposting to other services.
Contribute to the health profiling information and local data in line with key performance indicators.
To use professional knowledge and judgement to take into account the range of care/support packages and treatment options available. To consider the personal circumstances and preferences of children, young people and families in order to implement and adjust programmes of care and intervention.
To maintain and develop skills and knowledge to deliver a range of interventions and set an example of good practice to other team members including skill sharing.
To maintain sensitivity at all times to the emotional needs of children, young people, families & carers offering reassurance and empathy. This may be in regard to the nature/prognosis their condition/situation and implications of the same.
To work with individuals and carers who may find it difficult to engage with the service or aspects of the service resulting in uncooperative or challenging behaviour that has to be managed.
To use professional knowledge, skills and experience when working in changing environments and across professional boundaries.
To adhere to Staffordshire Safeguarding Children Board’s guidelines, to identify and report any case of suspected or confirmed abuse. To support any children and their families with identified health needs within the Child Protection and ‘Looked After’ system, contributing to reports in accordance with Child Protection policies and procedures.
To maintain contemporaneous and accurate records as required by the Trust and in line with professional standards (e.g. /NMC). This includes self-management and supervision of identified team members, giving guidance and direction where appropriate.
To take responsibility for the safe use and transportation of equipment between locations. To work in non-purpose built rooms involving the necessity to move furniture, therapy equipment etc.
To ensure that the skills of identified team members are appropriately deployed and that care is delivered by staff with the relevant education, experience and competence.
To deliver a high standard of intervention, whilst managing the demands of unplanned aspects of the role using innovative problem solving techniques where appropriate.
To provide advice to service users, carers and professionals regarding the management and care and support of children, young people, families and carers for example regarding continence and enuresis.
To provide professional and clinical support and leadership to identified team members.
To take accountability for allocated assessments, planning and implementation of care packages and programmes of support.
To provide and receive regular supervision in order to support the continuous professional development of self and others.
To contribute to the process of induction for new team members and other disciplines, as appropriate.
To identify own training/educational needs as part of the supervision and appraisal process. Attains and maintains an agreed level of expertise through ongoing training and development.
To have an understanding of people management skills which incorporate compassionate and appreciative leadership.
To set workload priorities for self, and oversee the workload of team members and students as required.
To provide training, undertake work based assessments and supervision of pre- registration students, typically for periods of 4-12 weeks as required.
To participate in and support the team in undertaking clinical audit.
To contribute to, and actively promote the implementation, development and upholding of policies, procedures, guidelines and directives, along with relevant statutory measures and governmental directives.
The successful applicant must have the ability to travel independently within the Trust.
