Full job description

This post is fixed term/secondment for 24 months due to meet the needs of the service

The successful candidate will join the Community Hospital Service based at Brecon Hospital, working within a supportive multidisciplinary team providing care for patients with frailty, rehabilitation and complex medical needs. The post offers an excellent opportunity to develop skills in community-based medicine, integrated care and the management of patients transitioning from acute hospital settings.

The post holder will work under the supervision of the Consultant Physician/ Specialist and will be supported to develop clinical, leadership and quality improvement skills. In keeping with Powys Teaching Health Board's integrated approach to care, there may also be opportunities to gain experience within Mental Health and Learning Disability services, depending on individual interests and service requirements, providing exposure to a broader range of community and integrated care pathways.

Early Help and Support – through development of Primary and Community Care services, including improved access.

Joined Up Care – a collaborative approach aimed at bringing care closer to home.
Workforce Futures – transforming the workforce to meet the needs of the population and the demands of the service.
Innovative Environments – delivering safe, effective, and sustainable physical environments, and developing new and innovative ideas.
Digital First – a conscious shift towards utilising new technologies.
Transforming in Partnership – embedding the principles of prudent and value-based healthcare.
