Care Co-ordinator – Medication Delivery & Prescription Support.
Coastal Primary Care Network (PCN) is seeking a caring, reliable and motivated individual to join our team as a Care Co-ordinator – Medication Delivery & Prescription Support.
This is a varied and rewarding role supporting patients across our community. The successful candidate will be responsible for delivering medications to housebound and vulnerable patients, while also providing administrative support within the practice and undertaking Prescription Clerk duties during periods of annual leave and staff absence.
The role offers the opportunity to build meaningful relationships with patients, support their healthcare needs, and become an integral part of a friendly and supportive primary care team.
No previous experience of SystmOne is required, as full training will be provided.
Main Duties and Responsibilities Medication Delivery
- Deliver medications safely and professionally to patients within the Coastal PCN area.
- Ensure medications are transported and handled in accordance with practice procedures.
- Maintain accurate records of deliveries.
- Liaise with patients, carers, pharmacies and practice staff regarding deliveries.
- Act as a professional representative of Coastal PCN and St Agnes Surgery.
- Report any concerns regarding patient wellbeing, safeguarding or vulnerability to the appropriate healthcare professional.
Care Co-ordination
- Support vulnerable, elderly and housebound patients to access healthcare services.
- Signpost patients to appropriate support services within the community.
- Support patient follow-up activities where required.
- Work collaboratively with the wider multidisciplinary team to improve patient outcomes.
Prescription Support
Following a structured training programme, the post holder will:
- Provide cover for Prescription Clerk duties during annual leave and planned absences.
- Process repeat prescription requests in accordance with practice protocols.
- Deal with prescription and medication queries from patients and pharmacies.
- Support clinicians and the prescription team with administrative tasks.
- Maintain accurate patient records using SystmOne.
- Ensure prescriptions are processed accurately and within agreed timescales.
Person SpecificationEssential Criteria
- Excellent communication and interpersonal skills.
- Strong organisational and time-management skills.
- Ability to work independently and as part of a team.
- Good attention to detail and accuracy.
- Ability to handle confidential information sensitively and professionally.
- Good IT skills.
- Full UK Driving Licence.
- Access to a reliable vehicle.
- Business-use motor insurance is essential and must be maintained throughout employment.
- Flexible and adaptable approach to work.
Desirable Criteria
- Previous experience in a healthcare, social care or customer service environment.
- Knowledge of GP Practice operations.
- Experience working with vulnerable or elderly patients.
- Experience using SystmOne.
- Understanding of repeat prescribing processes.

Additional Information
This role requires the regular use of the successful candidate's own vehicle for business purposes when delivering medications to patients.
The salary has been enhanced to reflect the requirement to use a personal vehicle for work-related travel and is intended to contribute towards fuel costs, business-use insurance requirements, and general vehicle wear and tear.

Pay: £13.50-£15.00 per hour
