Job Description

As a Care Planner within our Short Term Care at Home Service, you’ll play a vital role in ensuring our service users receive safe, effective, and person-centred care.

What you’ll do:

Complete and review care plans tailored to individual needs
Carry out risk assessments to ensure safe delivery of care
Work closely with care teams to maintain high standards
Support service users transitioning through short-term care
Ensure all documentation is accurate and up to date
Contribute to delivering high-quality, outcome-focused care

Qualifications

Organised and detail-oriented
Compassionate and person-centred in approach
Confident working independently and as part of a team
Experienced in care planning and risk assessments (preferred)
