As a Care Coordinator, you'll be the heart of keeping extraordinary care running smoothly. Working closely with your Care Manager, you'll support carers, clients, and the wider community making sure every visit is well planned, every team member feels supported, and every customer receives care they can truly rely on.

This role includes participation in our out-of-hours on-call rota, as required by the needs of the service.

What you'll do

Coordinate daily care rotas, ensuring visits are safely allocated, covered, and delivered on time.
Match care workers to service users based on skills, location, continuity, and individual needs.
Respond promptly to absences, missed visits, emergencies, and changes in care requirements.
Participate in the on-call rota (evenings, weekends, bank holidays), managing urgent issues and emergency cover.
Set up new care packages, ensuring all arrangements are in place before care begins.
Liaise with service users, families, care workers, healthcare professionals, and external agencies.
Keep care plans and schedules accurate, up to date, and communicated to staff.
Monitor electronic care and visit systems to confirm visits are completed as planned.
Maintain accurate records, including care documentation, incidents, safeguarding concerns, and medication records.
Support the Registered Manager with daily operations, care reviews, and quality monitoring.
Assist with recruitment, induction, and onboarding of new care staff.
Monitor staff compliance, including training, DBS checks, and Right to Work documents.
Carry out spot checks, care reviews, and quality assurance activities.
Report safeguarding concerns, incidents, complaints, and compliments per company procedures.
Provide hands-on care to cover visits during staff shortages or emergencies, following care plans and CQC standards.
Complete post-visit care records and report changes in service users’ needs or wellbeing.
Compile weekly staff hours and mileage reports for payroll and invoicing.
Support monthly staff meetings by recording attendance, updates, and follow-up actions.
Promote a culture of compassionate, person-centred, professional care.

What you'll bring

Level 3 Diploma in Adult Care (or equivalent) is essential. A Level 5 Diploma in Leadership and Management for Adult Care, or willingness to work towards it, is desirable.
Experience coordinating domiciliary care services.
Excellent organisational and time management skills.
Strong communication skills, confidence in decision- making and supportive, people-first approach
Confident using electronic care planning and rostering systems.
Good understanding of CQC Fundamental Standards, safeguarding, and person centred care.
Ability to remain calm and make sound decisions under pressure.
Full UK driving licence and access to a reliable vehicle is essential, as the role involves travelling between service users' homes and supporting care delivery across the local area.
