Job Summary:
The Clinical Pharmacist will be an integral member of the multidisciplinary healthcare team, working directly with patients and healthcare professionals to ensure the safe, effective, and economical use of medicines. Key Responsibilities:
1. Clinical Practice & Patient Care:
* Medicines Optimization: Conduct comprehensive structured medication reviews (SMRs) for patients with complex polypharmacy, multiple long-term conditions, and those in care homes, identifying and addressing medicine-related issues.
* Chronic Disease Management: Proactively manage patients with long-term conditions (e.g., diabetes, hypertension, asthma, COPD) in collaboration with GPs and practice nurses, including prescribing (if an independent prescriber) and monitoring.
* Acute Medicine Management: Respond to acute medicine requests, manage common ailments, and triage patient queries related to medication, signposting to other healthcare professionals as appropriate.
* Discharge and Transitions of Care: Facilitate medication changes and reconciliation for patients recently discharged from hospital, ensuring continuity of care and preventing medication errors.
* Medicines Information: Act as a primary point of contact for all medicine-related queries from patients and healthcare staff, providing evidence-based information and advice.
