Full job description

We are looking for a compassionate, patient, and reliable Support Worker to support a young boy with a brain injury and associated cognitive needs. The successful candidate will provide person-centred support with daily living, community access, and engagement in activities, while promoting his independence, confidence, and overall wellbeing.

Purpose of the Role:

The role is to provide consistent, person-centred support and appropriate prompting with daily living tasks and community engagement, enabling the client to live as independently and fulfilling a life as possible.

The successful candidate will have a calm, patient, and respectful approach and be comfortable supporting someone with cognitive challenges. You will also be required to help the client safely navigate his day, recognise and manage potential risks, and provide reassurance when needed.

Hours of work:

2 x 4-hour shifts per week on a Monday and Tuesday (8 hours per week)
Working hours will be after school, from approximately 15:00
There may be an opportunity for the hours to increase once a positive working relationship and level of trust have been established with the client.

Key Responsibilities:

Personal Care and Daily Living Support

Provide support and appropriate prompting around eating and monitor this as required.
Support the client in managing his learning disability and associated needs.
Encourage independence with daily routines and activities wherever appropriate.
Follow guidance and instructions provided by family members and relevant professionals.

Community Support

Accompany the client on outings within the local community, including physiotherapy activities and social activities.
Encourage and support participation in appropriate community-based activities.
Help the client safely navigate different environments and manage potential risks.
Promote confidence, independence, and positive engagement within the community.

Cognitive Support

Provide prompts, reminders, and reassurance with daily routines and tasks.
Use clear, simple, and patient communication, adapting your approach to the client's individual pace and level of understanding.
Maintain a calm, structured, and supportive environment to reduce confusion and anxiety.
Recognise and respond appropriately to changes in the client's behaviour, needs, or presentation.

Communication and Record Keeping:

Maintaining clear and concise records of support provided.
Communicating effectively with family members and relevant healthcare professionals regarding the client's well-being and any concerns.
Reporting any changes in the client's condition or needs to the appropriate individuals.

Person-Centered Approach:

Treating the client with dignity, respect, and empathy at all times.
Respecting the client's choices and preferences.
Encouraging the client's independence and promoting their well-being.

Person Specification:

Essential:

Previous experience in a care or support role, preferably supporting individuals with cognitive difficulties or brain injuries.
A genuine passion for supporting others and a commitment to person-centered care.
Excellent communication and interpersonal skills, with the ability to communicate clearly and patiently.
Ability to follow instructions and work independently.
Reliable, punctual, and trustworthy.
Respectful of confidentiality and professional boundaries.
Understanding of the challenges faced by individuals with cognitive impairments.
Willingness to undertake relevant training.
Enhanced DBS check (or willingness to obtain one).
Driver with own vehicle.

Desirable:

Formal care qualifications (e.g., NVQ/QCF in Health and Social Care).
First Aid certification.
Experience in supporting individuals within their own homes and in the community.
Knowledge of local community resources in Aylesbury
