Full job description

Job Overview
We are seeking a dedicated and compassionate Care Co-ordinator to join our team on a part-time basis, working 25 hours per week (Monday to Friday from 8am). The successful candidate will play a vital role in organising and overseeing care services within a domiciliary care environment, ensuring the delivery of high-quality senior care. In addition to coordinating care services, the post holder will deliver care and support to service users where required, ensuring continuity of care, responding to staff shortages, supporting new package starts, and maintaining high standards of person-centred care. This role offers an excellent opportunity for individuals with supervisory experience and strong leadership skills to make a positive impact on residents' lives while working in a supportive and professional setting.

You will also be expected to deputise for the RM occassionally.

Duties

Coordinate and oversee daily care activities for service users, ensuring personalised care plans are followed effectively
Supervise and support care staff, fostering a team-oriented environment with clear communication
Liaise with healthcare professionals, families, and external agencies to facilitate comprehensive care delivery
Organise schedules and allocate resources efficiently to meet residents’ needs
Maintain accurate documentation related to care plans, assessments, and incident reports using IT systems
Assist with the development and review of individualised care plans in accordance with best practices
Ensure compliance with health and safety regulations within the care environment
Support service users in maintaining their independence and dignity through respectful supervision and assistance
Deliver personal care, companionship, medication support, meal preparation, and other services in accordance with individual care plans.
Promote independence, dignity, privacy, choice, and wellbeing of service users.
Complete accurate care records and report any concerns promptly.
Observe and report changes in service users' health, wellbeing, or circumstances.
Follow all safeguarding, medication, infection control, and health and safety procedures.

Requirements

Proven supervisory or leadership experience within a domiciliary care setting
Strong communication skills, capable of liaising effectively with residents, families, and multidisciplinary teams
Valid driving licence is desirable for transporting residents or attending external meetings
Experience in developing or managing care plans is preferred
Proficient IT skills for maintaining accurate records and documentation
Knowledge of health and safety standards relevant to elderly or vulnerable adults’ care
Compassionate attitude with a genuine interest in providing high-quality senior care
Ability to work independently while demonstrating excellent organisational skills

This role is ideal for motivated individuals committed to enhancing the quality of life for older adults through effective coordination and leadership.

Pay: From £28,500.00 per year
